important features of medullary thyroid carcinoma

                       There will be swelling the anterior portion of the neck and with the enlarged thyroid gland. In medullary carcinoma thyroid, the TFT will be in normal limit. It is a multifocal or multicentric disease.
         The medullary thyroid carcinoma is a neuroendocrine origin which arises from the parafollicular C cells.

          The parafollicular C cells are the cells which are derived from the neural crest and these cells will secrete calcitonin.

           The main histological diagnostic feature of medullary thyroid carcinoma is an amyloid deposition. The medullary thyroid carcinoma can occur sporadically 80% and a familial form.

           The gene defect in malignant form is MEN - 2B and variable malignant is MEN - 2A. 

            The cervical lymph node is most probably affected first in the medullary thyroid carcinoma. The patient of MTC may have watery diarrhoea.

             The treatment is Total thyroidectomy and removal of affected lymph nodes by modified radical neck node dissection.
